Shawn,

If you're running an Avocent (or Cybex) SwitchView, try the following to 
do a "mouse reset":

Hit Ctrl-Ctrl-m-r (don't type the -'s) and hit enter.  If that doesn't 
work try the following.

Press and hold NumLock on the keypad (shift-key style) and hit - (also 
on the keypad) then type mr
