I'm surprised no one has mentioned VoicePulse yet. I cut my teeth on them. The cost was great and the service was good... and they let me hook the service up to my Asterisk server, so I could do some amazing things with the incoming calls (like send telemarketers into monkey-hell).

Skype is fine as a peer-to-peer network, but they loose more money/month than Vonage. They either have to change their pricing model - or disappear one day like Skyrocket...
