My suspicion is that the "modem" they gave you is a "modem/router", which is
giving your router a NATed IP via DHCP, which your router is in turn NATing.
This could cause apache to think that all requests are coming from the
internal IP of the cable modem's router interface. Most of those kinds of
devices have a web-based admin tool. If you can get the password for it then
you can usually turn off that device's router functions, and use your router
to do the NATing. I've seen this mostly in DSL modems, but according to what
I can find on Google about that series of cable modems, some do have built
in routers.
