AFAIK, the only tax return due to the IRS as a "sole proprietorship" is 
a Schedule C, attached to the 1040 that you already file for individual 
taxes. They do not require you to incorporate and you can file either 
under your SSN or a dedicated EIN. You can get an EIN by submitting form 
SS-4.
