While VoIP works great on the LAN, there is NO WAY I would run a 
business using strictly VoIP-on-the-Internet.

If you've got your own T1 straight to your provider (a la FeatureTel) 
you can make it work but if your voice packets hit the Internet you WILL 
regret it.

Stick with land lines for business use. Been there, done that, have the 
scars to prove it.
